This service is the direct delivery of health information, integrated into a trusted directory (Trusted Bundle), part of a long term national strategy to transition from paper-based to electronic health care records that can be shared more easily to reduce costs and improve the quality of patient care. SecureHIT Direct is designed to support the goal of health information exchange between providers using electronic health records (EHRs) engaged in Meaningful Use, the Medicare and Medicaid programs that help providers to pay for and meaningfully use EHRs.  It was also intended as a means of secure exchange between providers and patients. With SecureHIT Direct delivery in place, the data sharing needs of care coordination, transitions of care, and patient engagement can now be met at low costs, and with computer software familiar to anyone who has ever used email.  For example, Direct messaging is now being used in several communities to alert primary care providers when their patients are discharged from the hospital, and there are early indications that these alerts are lowering re-admission rates.
      
 

HOSPITALS

Coordination of Care - Use of a Certified EHR Techcnology to engage with patients or their authorized representatives about the patient’s care, sending and receiving patient massaging. 

 

Health Information Exchange - The eligible healthcare provider, provides a summary of care record when transitioning or referring their patient to another setting of care, receives or retrieves a summary of care record upon the receipt of a transition or referral or upon the first patient encounter with a new patient, and incorporates summary of care information from other providers into their EHR using the functions of certified EHR technology using SecureHIT as a HISP.

 

Public Health Reporting - The eligible hospital or healthcare provider should be in active engagement with a public health agency or clinical data registry to submit electronic public health data in a meaningful and interoperable method, using SecureHIT as HISP certified EHR technology, except where prohibited, and in accordance with applicable law and practice.

HEALTH ORGANIZATIONS AND SERVICE PROVIDERS

(Labs, Rads, Payers)

The costs of implementing traditional technologies to send and receive health information packages and maintain their security have increased significantly. SecureHIT offers you the ability to do so without binding to third parties or portals, allowing you to own the data.

Direct technology is integrated into more than 300 EHRs in Puerto Rico and the United States. This allows having only one tool to send and receive protected health information.

PUBLIC HEALTH REPORTING OBJECTIVE AND MEASURES

Objective: The Eligible Provider is in active engagement with a public health agency (PHA) or clinical data registry (CDR) to submit electronic public health data in a meaningful way using certified EHR technology (CEHRT), except where prohibited, and in accordance with applicable law and practice.
 

Measures: The public health reporting objective for Eligible Providers includes five measures. In Stage 3, Eligible Providers must attest to any combination.

 

Public Health Reporting: The eligible hospital or healthcare provider should be in active engagement with a public health agency or clinical data registry to submit electronic public health data in a meaningful way, using SecureHIT Direct delivery into a certified EHR technology the entity shall comply, except where prohibited, and in accordance with applicable law and practice.
